Surgical Procedure

The method of surgery is to tighten the skin of the abdomen by undermining the skin and stretching it. Excess skin and fat is then removed. The incision is placed across the lower abdomen but sometimes is like an upside down T. The scars are concealed by clothing. The operation is done under general anaesthetic.

Technique

This operation is done to tighten the loose skin of the abdomen and repair the weak muscles of the abdominal wall (rectus muscle). It is sometimes combined with liposuction to smooth the edges and improve the contour.

Surgical Procedure

Our surgeons prefer to make the incision in the crease of the armpit, since in most cases the scar is almost invisible. The incision is made with a special instrument designed to create a space for the implant. The extent of the incision must be smooth and appropriate for the size of the implant. After surgery, a bandage will be applied over your breasts for two days after surgery to speed up healing.

Types of Implants

All breast implants utilize a silicone shell but contain different kinds of filler, gel or saline. The outer silicone shell may be smooth shiny and polished, or a slightly rough texture. Breasts augmented with the textured implants are less likely to develop contracting capsules than those with the smooth implants. Placement of Implants

There are two techniques for placing implants.

Capsular contracture occurs if the capsules around the implants begin to tighten and thicken. This will cause the breasts to feel hard and appear unnatural. Hard breasts can be treated in several ways. Sometimes removal of the capsule is required, or perhaps the implant needs to be replaced. Bleeding is usually minimal during the operation. In an unusual case, bleeding after surgery may cause a large blood clot that needs to be removed. Infection is an uncommon risk. However, if a breast becomes infected, the implant must be removed immediately. You need to wait for several months before repeating the operation.

There are many ways to perform a breast lift. The method must be tailored to the patient. A common method of surgery is to mark the new nipple position. Then with the blood supply of the nipple preserved on a pedicle of tissue, the excess breast is removed. The nipple is then moved into its new position and the new breast shape is reconstructed. The incision is often around the nipple and on the under surface of the breast, like an upside down T. The operation is done under general anaesthesia.

Patients who receive this procedure usually remain in the hospital over night. If the breasts are not too large and the skin has good elasticity then liposuction may be possible. An incision is made just beneath the lash line. Excess fat, muscle and skin are removed. Fine sutures are used to close the incision. Permanent stitches will be removed 3-5 days after the procedure.

The upper eyelid incision is made in the natural skin fold. Post Operative Care

For the first week, you'll need to avoid activities that dry the eyes, including reading, watching television, wearing contacts, and using a computer. Also avoid excessive blinking, which leads to increased swelling. You should also wear dark sunglasses for a couple of weeks to protect your eyes from wind and sun irritation. The eyes may tire easily for several weeks; frequent naps are helpful.

For the first three or four weeks, you should avoid any activity that increases blood flow to the eyes, including bending, lifting, crying and sports. Your surgeon will let you know when you are ready for exercise. Also avoid drinking alcohol, which can lead to fluid retention and delay recovery.

Surgical Procedure

The incisions are placed on the rim of the alar of nose. Soft tissues of the nose are then separated from the underlying structures, the bone and cartilage are then reshaped and the skin re-draped over the surface. Patients who have a deviated septum may benefit by correction of this problem at the time of surgery.

When rhinoplasty is performed by a qualified plastic surgeon, complications are infrequent and usually minor. Nevertheless, there is always a possibility of complications, including infection, nosebleed or a reaction to the anaesthesia. You can reduce your risks by closely following your surgeon's instructions both before and after surgery.

After surgery, small burst blood vessels may appear as tiny red spots on the skin's surface; these are usually minor but may be permanent. As for scarring, when rhinoplasty is performed from inside the nose, there is no visible scarring at all; when an "open" technique is used, or when the procedure calls for the narrowing of flared nostrils, the small scars on the base of the nose are usually not visible.

After Care

The nose will be covered with either plaster of Paris, or a light metal or plastic splint to maintain the position of the nasal bones.

Recovery

Most rhinoplasty patients are up and about within two days, and able to return to school or sedentary work a week or so following surgery, however full recovery may take several weeks.

Results

Your nose will begin to look better and your spirits will improve. Within a week or two, you'll no longer look as if you've just had surgery.

Avoid strenuous activity (jogging, swimming, bending, sexual relations-any activity that increases your blood pressure) for two to three weeks. Avoid hitting or rubbing your nose, or getting it sunburned, for eight weeks. Be gentle when washing your face and hair or using cosmetics.

LASIK Centre

Wavefront-guided LASIK and PRK, as used at the LASIK Centre at Bangkok Hospital Phuket, is a new technology that provides an advanced method for measuring optical distortions in the eye. The Wavefront system works by measuring how light is distorted as it passes into the eye and then is reflected back. This creates an optical map of the eye, highlighting individual imperfections. As a result, surgeons can customize the procedure according to each individual patient's unique vision correction needs.

LASIK Eye Surgery

With LASIK, a Microkeratome laser is used to shave the cornea to create a flap. The flap then is opened to expose tissue just below the cornea's surface. Re-shaping of the cornea to treat nearsightedness, farsightedness or astigmatism is then performed by removing ultra-thin layers with the use of the Excimer laser. Finally, the flap is returned to its original position, without sutures.

The vast majority of LASIK patients only feel mild discomfort. A clear, light plastic shield is placed on your eye(s) following surgery. This is worn for the rest of the first day, and every night for one to two weeks. It prevents you rubbing your eye(s) in your sleep. You will also be given eyedrops to use for the next few days.

PRK - Photorefractive Keratectomy

PRK differs from LASIK in that no corneal flap is created prior to the laser treatment. During PRK eye surgery, the surgeon accesses the cornea's inner layers by removing the thin outer layer of the cornea, the epithelium, which has been softened with anesthetic eye drops. The Excimer laser is then positioned over the center of the cornea and changes the shape of the cornea by removing a tiny amount of corneal tissue. This part of the procedure can take less than a few minutes for most patients. Then the surgeon will place a bandage contact lens on the eye. This contact lens is left on the eye for a few days while the epithelium grows back over the surface of the cornea.

Having had LASIK you will appreciate your vision getting clearer, hour by hour, for the rest of the day after your treatment. However, a very significant improvement will be noticed the next day. PRK usually takes a little longer (3-7 days).

Patients who wear contact lenses are asked to stop wearing them in order to allow time for the shape of your cornea to revert back to it's natural shape. This then means that an accurate measurement can be taken of the amount of corneal tissue to remove.
